About The Position

The Housing Specialist - Community Health Worker at UnitedHealthcare is responsible for overseeing housing initiatives and improving access to affordable housing for Medicaid members in Florida. This role involves developing and implementing housing strategies, building partnerships with housing providers, and assisting individuals in securing accessible housing. The position allows for telecommuting, providing flexibility while addressing significant challenges in the community.

Requirements

  • High School Diploma/GED (or higher)
  • 3+ years of experience managing needs of complex populations (e.g., Medicare, Medicaid)
  • 3+ years of experience in a community health setting
  • 1+ years of community case management experience
  • 1+ year of experience with the homeless population and housing
  • Reside in the state of Florida

Nice To Haves

  • Minimum 1-2 years of experience in healthcare, social work, public health, or related field, OR bachelor's degree in healthcare
  • Minimum 1-2 years of relevant experience in social work or case management
  • Experience in a social service setting with knowledge of case management systems

Responsibilities

  • Research, establish, and maintain a list of available housing opportunities in the region
  • Develop relationships with local community resources including transitional housing, shelters, and permanent housing
  • Develop individualized housing plans and communicate them to members
  • Assist members with their housing search, including applying for apartments and negotiating with landlords
  • Help members understand tenant rights and responsibilities
  • Act as a mediator for members in case of disagreements with landlords
  • Conduct follow-up meetings with members after housing placement
  • Maintain accurate documentation of service objectives and outcomes
  • Participate in collaborative team meetings to provide input and develop best practices
  • Assist members with in-person assessments or housing tenancy
  • Perform other duties as assigned or required
