About The Position

The Housing Solutions Case Manager provides comprehensive case management services to individuals and families, supporting them with housing, employment, financial planning, and access to community resources. This role includes developing individualized housing plans, maintaining detailed case documentation, advocating for clients, and coordinating with landlords and support agencies. The Case Manager ensures clients maintain housing stability and comply with program policies, working collaboratively with team members and external partners to achieve program goals.

Requirements

  • Computer literacy and proficiency in Microsoft Office applications is required
  • Valid CA driver’s license, proof of insurance, and reliable vehicle required; must be comfortable using personal vehicle for home visits, shelter visits, and community meetings
  • Ability to work with a diverse population; organize and prioritize tasks; speak effectively in small groups; manage crisis situations; and maintain professional, respectful communication
  • Must be dependable, responsible, and able to maintain positive and effective working relationships
  • Strong organizational, communication, and creative problem-solving skills are desired

Responsibilities

  • Manage a caseload of 17–25 clients, meeting with each at least twice per month in shelters, their homes, or community settings
  • Develop individualized housing support and emergency response plans based on assessments and identified barriers, including both short- and long-term measurable goals
  • Deliver comprehensive case management services such as needs assessments, benefits coordination, housing planning, tenant education, advocacy, and referrals
  • Design and monitor personalized case management plans centered on housing stability and financial goals
  • Document client progress through organized case files, charts, monthly notes, and outcome evaluations
  • Assist clients with housing and employment challenges, recognizing strengths while addressing needs in a respectful manner
  • Conduct home visits and inspect residences to ensure they are well-maintained
  • Stay informed about community resources and provide relevant referrals
  • Follow all Case Management policies and maintain a full caseload
  • Participate in the Coordinated Entry System (CES) by submitting updates, requesting matches, and attending match meetings
  • Help clients identify housing opportunities and mediate landlord-tenant relationships
  • Build and maintain strong relationships with landlords and supportive service providers
  • Collaborate with team members and volunteers to support Mercy House’s mission and values
  • Regularly attend internal and external meetings
  • Assist in the planning and delivery of resident programs
  • Demonstrate adaptability, dependability, and empathy in a fast-paced team environment
  • Communicate effectively and respectfully with individuals from diverse backgrounds
  • Keep client documentation accurate and up to date, including case notes and stabilization plans
  • Show proficiency with Microsoft Office applications
  • Contribute to the accuracy and effectiveness of data tracking systems
  • Handle client files and information with confidentiality and care
  • Ensure timely submission of all necessary client documents
  • Perform additional duties as assigned
