About The Position

The Housing Solutions Case Manager, under the direction of the Housing Solutions Manager, is responsible for conducting ongoing needs assessments; developing personal housing and emergency plans; and providing implementation, support, information, referrals, and advocacy for individuals in the Mercy House Permanent Supportive Housing Program. This role requires building strong working relationships with residents to support their ability to increase self-reliance. The Case Manager will recognize and reinforce positive behavior changes while identifying challenges that may threaten residents’ self-reliance and housing stability. Maintaining clear and accurate files and records is essential. This position practices trauma-informed, client-centered strategies.

Requirements

  • Bachelor’s degree in Social Work or a related field preferred, with experience in chemical health, mental health, and co-occurring disorders
  • Candidates without a bachelor’s degree may apply if they have at least 2 years of direct experience working with individuals experiencing long-term homelessness, low-income populations, and diverse communities, along with knowledge of mental health and addiction issues
  • Valid California driver’s license, proof of insurance, and a reliable vehicle required
  • Willingness to use a personal vehicle for home visits, shelter visits, and community-based meetings
  • Ability to work effectively with diverse populations; plan, organize, and prioritize tasks; perform crisis intervention; communicate clearly both verbally and in writing; and maintain a professional and safe environment
  • Strong dependability, responsibility, and respectful communication skills
  • Ability to lift up to 50 pounds
  • Ability to sit for extended periods while working at a computer

Responsibilities

  • Maintain a caseload of 20–25 individuals and meet with residents at least twice per month to develop emergency plans and individualized housing support plans based on assessments that identify barriers, including short- and long-term measurable goals
  • Utilize harm reduction approaches to provide client-centered services that promote housing stability
  • Apply crisis prevention and intervention skills
  • Practice trauma-informed care and motivational interviewing techniques
  • Implement eviction prevention strategies to support clients in maintaining housing
  • Support residents in building on their strengths while identifying needs in a respectful manner
  • Conduct home visits and inspect units to ensure proper maintenance
  • Maintain knowledge of community resources and provide appropriate referrals and linkages
  • Assist in facilitating social and recreational activities that promote community involvement and peer support
  • Collaborate with volunteers and community partners to connect residents with services such as employment, education, tutoring, and mentoring
  • Attend trainings to enhance knowledge in areas such as co-occurring disorders, harm reduction, and cultural competence
  • Facilitate relationship-building between landlords/caretakers and residents
  • Participate in the emergency on-call schedule to ensure client needs are met
  • Collaborate with staff and volunteers to implement Mercy House’s mission and core values
  • Attend regular internal and external meetings
  • Contribute to planning and development of resident programs
  • Maintain accurate, up-to-date documentation for all clients, including housing plans, emergency plans, case notes, assessments, call logs, and Housing Authority documentation (applications and recertifications)
  • Complete all required paperwork in a timely and accurate manner to ensure proper agency reimbursement
  • Assist with data collection systems
  • Develop knowledge of Adult Rehabilitative Mental Health Services and substance use treatment
  • Perform other duties as assigned
