About The Position

Chartiers Center is a Community Mental Health/IDD Center that has a rich range of service offerings. Among them are the highly ranked Hestia, Atlas and Vesta Programs that are part of our Homeless Housing Specialty Services. We are seeking a talented and compassionate individual to join our team as a Housing Coordinator to change the lives of the people we serve. Do you want to be a life changer? Housing Coordinators change lives by: Work with individuals experiencing homelessness to help them identify a place to live and to develop the skills and supports to prevent relapse to being homeless, Act as a liaison between landlords and clients Provide ongoing case management and referrals to assist clients with becoming self-sufficient Work independently in the community and in client homes while still being a part of a team. change lives through, Work with treatment teams to assist with recovery. Requirements

  • Associate's Degree in Human Services and 2 years experience working with individuals experiencing homelessness.
  • Valid Driver’s License

Responsibilities

  • Work with individuals experiencing homelessness to help them identify a place to live and to develop the skills and supports to prevent relapse to being homeless
  • Act as a liaison between landlords and clients
  • Provide ongoing case management and referrals to assist clients with becoming self-sufficient
  • Work independently in the community and in client homes while still being a part of a team
  • Work with treatment teams to assist with recovery.
