This position is responsible for determining eligibility for participation in housing programs for Benton and Franklin Counties. Responsible for education and advocacy about available programs and performing assistance throughout the process of obtaining and maintaining stable, permanent housing for underserved and at-risk community members. The Housing Program Navigator is also responsible for documenting case notes and program activities, entering data and preparing monthly reports, tracking financial data, and assisting with annual program progress reports.

  • Provides ongoing case management to individuals currently in the Housing Programs. Case management includes, but is not limited to: screening, intakes, assessment, service plan development, monitoring, linkage to appropriate community resources, follow up, appropriate progress tracking, assistance with independent living skills, etc.
  • Assists participants in completing lease agreements and obtaining other supportive documents such as identification, social security cards, and applications for relevant social service or other housing programs.
  • Maintains up to date information regarding tenant occupancy, move outs, vacancies, and other property management issues.
  • Assists the team in tracking and collecting and compiling data into the HMIS data system.
  • Facilitates and coordinates supportive activities with other agencies, including accessing mental health, substance abuse, or medical services, employment assistance activities such as job readiness skills, and any other relevant activities for program participants.
  • Maintains thorough and complete records of participant's activity and progress.
  • Establishes and maintains collaborative working relationships with community resources and stakeholders.
  • Mediates disputes between participants and landlords.
  • Attends appropriate service provider and community resource meetings.
  • Attends training and workshops as directed.
  • Assists with developing strategies to inform the community about the programs and to improve service delivery.
  • Enters, scans, updates, and maintains information in spreadsheets, databases, and reports.
  • Maintains electronic filing, paper filing, and record systems to provide easy access to records and information; provides retention of records as required by Department and County policies.
  • Performs other duties as assigned.
  • Associate's or Bachelor's degree from an accredited college or university in social services or related field.
  • Three years of social services experience with special needs populations (combined with Associate's degree) or one year of experience (combined with Bachelor's degree) in social service environment OR any combination of education and experience which would provide the knowledge, skills, and abilities to perform the job.
  • One year of experience in adult case management and/or housing programs required.
  • Valid Washington State Driver's License if driving a vehicle for County business.
  • Knowledge of how to determine eligibility for participation in housing programs for Benton and Franklin Counties.
  • Knowledge of the programs, community outreach, providing admissions procedures, performing intakes, monitoring client's progress, and needs, accessing and linking clients to community resources, and helping clients to successfully transition to permanent, stable housing.
  • Knowledge of administrative policies and procedures of the County.
  • Knowledge of The County's and Department's operating requirements, policies, procedures, and practices; and local, state, and federal regulations related to department programs and operations.
  • Knowledge of current office practices and procedures.
  • Knowledge of operation of standard office equipment and software consistent with this position.
  • Skill in organizational and time management to prioritize duties to accomplish a high volume of work product while adapting to constant changes in priority.
  • Skill in case and program activity documentation; reporting and financial data tracking; annual progress reporting.
  • Skill in de-escalating situations.
  • Ability to interpret and implement local policies and procedures; written instructions, general correspondence; Federal, State, and local regulations.
  • Ability to read, write, speak, understand, or communicate in English sufficiently to perform the duties of this position.
  • Ability to prepare and maintain accurate and concise records and reports.
  • Ability to apply sound judgment and discretion in performing duties, resolving problems and interpreting policies and regulations.
  • Ability to work with homeless and low income individuals of diverse backgrounds, cultural identities, and disabilities including physical, mental, substance abuse, HIV/AIDS, etc.
  • Ability to communicate detailed and often sensitive information effectively and concisely, both orally and in writing.
  • Ability to maintain professionalism at all times.
  • Ability to maintain effective working relationships with individuals within and outside the organization.
  • Ability to maintain confidentiality and discretion regarding business-related files, reports and conversations, within the provisions of the Public Records Act and other applicable State and Federal statutes and regulations.
  • Ability to work the allocated hours of the position.
  • Bilingual in Spanish preferred.
