The Housing Navigator functions as a facilitator between the Street Medicine, Care Management departments and Outpatient clinics with regard to the coordination of homelessness resources. Under the direction of Social Services and Street Medicine leadership, the Housing Navigator assists MLK staff with linking patients experiencing homelessness with homeless resources throughout the community, and is responsible for building relationships with community-based organizations that will best serve Martin Luther King Jr. Community Hospitals' patients and their families.
