In this position, you will provide Housing Support Services to Medicaid service consumers in need of necessary supports to obtain and reside in an in-home setting.   • Evaluate service needs and develops individualized Housing Support Services assistance plan based on factors such as psychosocial, cultural, financial, and medical conditions.
  • Provide Housing Support Services for individuals either through consultation with the consumer's Medicaid Case Manager or in working directly with the individual.
  • Provide counseling related to crises in individual's condition, and helps individuals and families make decisions regarding issues surrounding placement to maximize the functioning of the client's natural support system.
  • Screen for assistance through a referral from a Medicaid Case Manager and will follow up with family members, providers, physicians for additional information and confirmation, as needed.
  • Assess for the need of the service based on the following information and criteria: functioning capacity with behavior, coping abilities, injury disability, condition, and needs; and support systems available for service delivery.
  • A valid driver's license and acceptable driving record are required for this position.
  • Six months of experience providing services to persons who are elderly or disabled AND
  • Training and experience which shows knowledge of at least four (4) of the areas listed:
  • medical and psychosocial problems of the elderly and disabled.
  • normal and abnormal human development and behavior.
  • appropriate intervention methods for the elderly and disabled.
  • medical terminology and procedures.
  • implications of illness or injury upon clients and families.
  • State and Federal rules about long-term care, medical services, and clients' rights.
  • policies and procedures about agency programs.
  • available community resources.
  • crisis intervention, counseling, being an advocate, community relationships, and/or referral methods.
  • RN or MSW desirable but not required.
  • Experience communicating effectively with individuals from diverse backgrounds, especially when explaining program guidelines and responding to questions.
  • Experience providing active listening to people, interpreting their needs, and translating those into appropriate actions, solutions, or referrals.
  • Experience in working with older adults or individuals with physical disabilities with an understanding of their unique challenges, rights, and support requirements.
  • Experience building collaborative relationships with community partners.
