Under minimal supervision of the RESET Program Manager, the Housing Navigator will work independently to assist participants in identifying, securing, and maintaining safe and affordable housing. The role involves developing individualized housing stability plans tailored to participant needs, supporting participants in applying for rental assistance and subsidies, and providing ongoing follow-up to ensure housing stability and address barriers. Additionally, the Housing Navigator will deliver wraparound case management services, connecting participants to supportive resources such as employment, education, treatment, and healthcare. Regular check-ins with participants will be conducted to monitor progress and update housing plans, while collaborating with the Program Manager on substance use disorder (SUD) counseling referrals and relapse prevention strategies. The position also requires establishing and maintaining strong relationships with landlords, housing providers, and community partners, advocating on behalf of participants with housing agencies and service providers, and participating in community outreach and program-related events. The Housing Navigator will maintain accurate and timely documentation of participant services, housing progress, and case notes, inputting data into case management software to track outcomes and ensure compliance with program and funder requirements, and assisting in preparing program reports as needed. Regular travel to participant residences, landlords, and community partners is required, using personal means of transportation with reimbursement provided after the fact.
