About The Position
The Housing Coordinator plays a vital role in the Transition Age Youth Program, which serves current and former foster youth, probation placements, and unhoused youth aged 15-25. This position is centered around providing coordination of care, counseling, and housing navigation services to these young individuals. The Housing Coordinator is responsible for establishing positive and professional relationships with youth, assisting them in finding, securing, and maintaining housing within the community. This includes providing comprehensive case management services and trauma-informed mental health support. The coordinator acts as a liaison with landlords, outside agencies, and community partners to facilitate the housing process for the youth. In addition to housing navigation, the Housing Coordinator will conduct comprehensive intake assessments and goal-setting sessions with clients. Regular meetings, both weekly and monthly, will be held to evaluate clients' progress towards their goals and assess their eligibility for the housing program. The coordinator will also provide weekly counseling sessions aimed at building coping strategies and resilience among the youth. Creating and facilitating life skills workshops is another key responsibility, as is connecting youth with necessary referrals to outside agencies to help them achieve their goals. The role requires tracking and ensuring that clients' rent, allowances, and reimbursements are processed on time. The Housing Coordinator will also be expected to work in the drop-in center one day per week, if needed, and will have an on-call shift approximately once every 4-6 weeks. Knowledge of trauma-informed care, motivational interviewing, crisis intervention, and community resources is preferred but not mandatory. Familiarity with the Santa Cruz County rental market and social media housing platforms is also beneficial. Experience in a non-profit setting and with MediCal billing and electronic medical records is advantageous.
