About The Position

Manage service and care provision for residents in Congregate and Supportive Housing sites. Activities include recruitment and assessment of applicants, community outreach, service planning and coordination, provision of direct individual and group services, development of congregate "community", participation in multidisciplinary teams.

Requirements

  • The position requires an individual who is a self-starter with a positive and enthusiastic attitude as well as excellent oral and written communication skills.
  • Must be able to work independently as well as part of an interdisciplinary team and have the ability to problem solve and provide resource and referral options to residents.
  • Computer proficiency required.
  • Knowledge of housing issues, service delivery systems for elders and persons with disabilities, and understanding of the aging process and disability issues, ability to establish and maintain trust and working relationships with consumers, families, staff, housing and other community agencies.
  • Requires: valid driver's license and insured automobile in good working condition; ability to visit with consumers in their own home, availability to work between the hours of 9:00 a.m. to 5:00 p.m., availability to work in the event of an emergency during off hours.

Nice To Haves

  • Bachelor’s Degree in a Human Service related field with a minimum of two years experience working with elders, individuals with disabilities or caregivers preferred.

Responsibilities

  • Recruit applicants for congregate housing through community education and outreach.
  • Facilitate Multi-disciplinary Assessment Team (MAT) as required
  • Tenant selection procedures;
  • Tenant application form;
  • Tenant termination process.
  • Develop and implement a supportive service plan for each resident including:
  • Identifying the supportive service needs and resources of each tenant;
  • Linking each tenant with appropriate community resources;
  • Reassessing the supportive service needs of tenants and revise the supportive Service plan as needed.
  • Coordinate the delivery of supportive services to residents of congregate housing.
  • Facilitate open and ongoing communication, problem-solving and conflict resolution among residents.
  • Integrate the residents into the community by:
  • Alerting residents to activities and events in the community;
  • Encouraging the use of the community space (for example, mealsite, senior center) in the congregate housing facility by others;
  • Providing information to the public about congregate and supportive housing.
  • Other duties as assigned by immediate supervisor or member of Management Team.
