About The Position

The Housing Case Manager contributes to AHF by providing compassionate, client-centered case management that ensures individuals and families experiencing housing instability are connected to safe, stable, and supportive housing. Through consistent coordination, timely documentation, and compliance with program benchmarks, the Housing Case Manager supports participants in achieving housing stability, increasing income, and managing their health. By navigating services such as lease processing, benefits enrollment, and medical linkage, this role plays a critical part in advancing AHF's mission to reduce disparities and improve quality of life for people living with or at risk for HIV. The Housing Case Manager enhances the reputation of AHF by delivering high-quality, timely, and affirming support to clients navigating housing instability, chronic illness, and systemic barriers. By maintaining consistent contact, accurate documentation, and compliance with program goals, the Case Manager demonstrates AHF's commitment to accountability and excellence. Their ability to build trust with participants, respond quickly to challenges, and connect individuals to housing, benefits, and healthcare reinforces AHF's reputation as a reliable, client-centered organization that upholds dignity and care for those most impacted by health and housing inequities.
