The primary role of the Housing Case Manager is the ongoing support of clients within a permanent supportive housing environment. The Housing Case Manager engages clients at the housing site and in the outside community to orient to the expectations of supportive housing, develop an ongoing Individual Support Plan, and to assist with linkage to needed community resources. The goal of the programs is to improve individual and community outcomes related to safety, holistic wellness, recovery, increase in skills and income, and housing stability. The Housing Case Manager plays an integral role in assisting clients in developing skills across a variety of life domains through teaching, linking, and coaching. They engage clients to problem-solve conflicts and emerging crises that may destabilize and impair progress toward individual goals by engaging clients in positive, strengths-based, productive encounters. In addition to individual interactions with clients, the Housing Case manager plays a critical role in cultivating a harmonious and safe community. Day-to-day Success as a Housing Case Manager is a result of innovation and creativeness, flexibility, and a willingness to apply best practices and experiences effectively. Housing Case Managers must have flexible work schedules to reflect the needs of a 24-hour program. The ability to engage clients in both a planned and drop-in manner is key and effective time management and organizational practices are a must. They must be able to balance attention to the long-range goals of clients with day-to-day attention to the small, concrete steps clients must take to reach these goals. They must be able to assess and advocate individual client’s needs within the wider context of a housing community that requires clients to fulfill expectations such as timely payment of rent, apartment care, and neighborliness. They must be able to assess and deescalate conflict with clients in a manner that ensures safety for the individual as well as the community Housing Case Managers must be willing to devote the time and energy required to ensure that all clients are engaged and that all encounters with clients are positive, professional, and reinforcing of the key missions of the program as well as the agency. The Housing Case Manager must possess self-confidence, creativity, and the ability to exercise discretion and judgement autonomously.

Requirements

  • Current and valid driver's license, with a clean driving record
  • Associate’s degree and 2+ years of experience and/or lived experience in human services
  • Bachelor’s degree in human services or related field

Nice To Haves

  • 1+ years of professional experience in human services
  • Intermediate computer skills which include a moderate understanding of operating systems, word processing, spreadsheets, databases
  • Ability to relate to people from diverse backgrounds with multiple individual and systemic barriers to success
  • Excellent verbal and written communication skills required
  • The capability of working alone with very little supervision

Responsibilities

  • Provide Person Centered Case Management, Client Skills groups and Community Supports.
  • Develop and maintain a working knowledge of available community resources.
  • Act as a role model for clients by exhibiting productive behavior and conducting service interactions with a high level of professionalism and boundaries.
  • Administer UA drug tests in accordance with a client’s individual support plan.
  • Maintain electronic health records in accordance with the expectations for timely and thorough billing for services rendered.
  • Safeguard personally identifiable health information (PHI) in accordance with duties and current agency HIPAA notification statement and standards.
  • Flexible schedule to meet case load need, including work a minimum of one evening per week
