About The Position

The Housing Case Manager will provide comprehensive case management services to participants in a supportive housing program. This role focuses on assessing client needs, developing individualized service plans, and connecting clients with necessary resources to achieve housing stability and economic security. The Case Manager will conduct home visits, maintain case notes, and utilize ClientTrack software for program data management.

Requirements

  • Bachelor's degree in social work, psychology, sociology, counseling, or related service/science or health care related disciplines.
  • Minimum of two years' experience with homeless programs or social services.
  • One year of experience providing case management services.
  • Knowledge of community resources.
  • Excellent oral, written, and interpersonal skills.

Responsibilities

  • Conduct home visits for housed clients, ensuring a minimum of two contacts per month, including one in the client's home.
  • For unhoused clients, maintain a minimum of four contacts per month during the first 90 days, including three face-to-face contacts and one other form of communication.
  • Identify and articulate critical issues confronting clients and assist in developing goals and identifying resources.
  • Conduct program intake and assess clients' housing barriers, needs, income, and preferences.
  • Develop service plans in coordination with participants to locate safe, affordable housing.
  • Connect clients to relevant referrals and community resources.
  • Assist with outreach and negotiation with landlords/property owners.
  • Support clients in submitting rental and job applications.
