Housing Case Manager & Social Services Coordinator

Responsibilities
- Assists clients in achieving their housing goals
- Develops a person-centered service plan for each client
- Assesses each client's needs, with an emphasis on strengths and barriers related to getting & keeping their housing
- Works with property management staff to help clients maintain their housing
- Identifies strategies that will help clients maintain their housing and strengthen their recovery
- Coordinates/facilitates speakers, groups or events that foster community, identify helpful resources, and reinforce wellbeing
- Supports clients in understanding the basic expectations of being a tenant, lease requirements, household maintenance, etc.
- Collaborates with landlords and other community partners, advocating for client needs and rights.
- Assists clients in applying for rental assistance, SNAP benefits, social security income, etc.
- Participates in regular supervision and team meetings
