The position involves providing case management services as part of a multidisciplinary team. The primary responsibilities include acting as the point person to mitigate housing issues, developing treatment plans with consumers, and serving as the primary case manager in accordance with Eliot's policies and PACT standards. The role also requires participation in assessments, evaluating clients' functions, strengths, and needs, and developing therapeutic connections to provide clinical interventions. Additionally, the position includes providing problem-solving supports and interventions to clients, as well as being available for on-call duties on a rotating basis and working rotating evening shifts.
