The Outpatient Program is specifically designed for people who struggle with substance use disorders, mental health issues or co-occurring disorders. The Outpatient Program works with high-risk populations from a variety of backgrounds. Our programs are open and welcoming to adult participants, of any gender identity. We accept the majority of participants who seek Outpatient treatment, including walk-in’s, referrals from other agencies and step-down transitional clients from residential treatment programs. The Housing Case Manager II is a hybrid role that merges housing navigation with recovery-focused case management. This position supports clients across residential, outpatient, and community settings to maintain housing stability, engage in treatment, and transition successfully into independent living. Services are delivered in alignment with Drug Medi-Cal (DMC) billing requirements, including unbundled residential case management and outpatient care coordination. The Housing Case Manager II conducts field-based work with housing authorities, landlords, and community partners while collaborating closely with clinical teams to ensure continuity of care.
