The primary purpose of this position is to work directly with Money Follows the Person (MFP) Transition Teams, property management; local public and private aging, disability and homeless provider agencies; hospital discharge workers; and Managed Care Organizations (MCOs.) The position will help to further the Department's agenda on implementing evidence based best practice in supportive housing for a range of populations as part of the Department's Olmstead Plan implementation. This position is a field-based position that will serve an assigned region of the state that reports to the Section Chief of Housing and Homeless Programs. The duties of this position are as follows: Review and process referrals, review vacancy notices from properties and available subsidies to match with people with disabilities in need of housing, including but not limited to Money Follows the Person participants, Transitions to Community Living (TCL) participants, homeless individuals with disabilities, and the general population of individuals with disabilities. Manage the Wait/Interest List for available DHHS Housing Resources. Work with Targeting Program systems by reviewing vacancies and matching referred individuals with units that meets their needs, enter data on available properties, property contact information, referral agencies, and property wait lists. Handle phone calls, emails and other communications to provide program guidance and technical assistance to stakeholders on housing resources and Fair Housing Rules. Serve as liaison between referral entities and property management companies regarding Targeting applicants and tenants including assisting with conflict resolution and problem solving for applicants for housing and current tenants of housing when tenancy crisis issues occur to assist with resolution. Participate in Housing Team Meetings to maximize effectiveness of DHHS Housing Programs, identify areas for program growth, suggest ideas for improvement and collaborate with team members to determine if policy changes are needed including (in person/virtual staff meetings, Peer to Peer Technical Assistance, strategic planning of program changes related to new initiatives, professional development, and relevant workshops and other training activities. Ensure Targeted Unit applicants and property managers have a thorough understanding of Targeting Program and Key Rental Assistance program policies and procedures. Assist with the development of trainings for property managers, both face to face and virtually. Assist with the tracking of staff turnover among stakeholder organizations, including property management and referral agencies, to ensure current staff are appropriately trained. Serve as a resource and provide technical assistance to increase the knowledge of our partners in housing and human services systems with an emphasis on improving the capacities of entities that are supporting individuals to transition out of institutional settings, including but not limited to provider agencies, key stakeholders, and partner agencies.)

  • Bachelor's degree in public service administration, psychology, sociology, or social work or a human service programmatic field, from an appropriately accredited institution; or Bachelor's degree from an appropriately accredited institution, and two years of administrative or consultative experience in a human service program; or an equivalent combination of education and experience.
  • Good working knowledge of housing and homeless programs, barriers to accessing and maintaining housing, and community based supportive services needed to assist individuals in overcoming these barriers.
  • Knowledge of the concepts of affordable housing policy and fair housing law. As it relates to expanding affordable and accessible supportive housing options, this position should also have knowledge of permanent supportive housing for adults with disabilities and the ability to transfer that knowledge into programs.
  • Understanding, interpretation and applicability Federal and State Rules, Regulations and procedures associated with various low-income housing finance and supported housing programs.
  • Skilled in analytical thinking, problem solving, change management; meeting facilitation; attention to detail, judgment, data entry, accountability, coordination of work and operations, and decision making.
