About The Position

The HouseCalls Nurse Practitioner role at Optum involves conducting in-home health assessments for enrolled Medicare Advantage members. Requirements

• Active NP license and national certification (ANCC or AANP) or ability to obtain them in the state of assignment.
• Family, Geriatrics, Adult, or Adult-Gerontology Primary or Acute Care certification.
• Access to reliable transportation for travel to client and patient sites.

Nice To Haves

• 1+ years of clinical experience in a relevant clinical setting.
• Experience in gerontology, cardiology, internal medicine, or endocrinology.
• Home health care or home visit experience.

Responsibilities

• Conduct in-home assessments on health plan members, averaging 45-60 minutes per visit.
• Perform and complete assessments electronically, including past medical history, review of systems, and medication review.
• Conduct evidence-based practice screenings and check vital signs, performing physical exams and point of care testing as appropriate.
• Identify diagnoses for care management and active medical management based on assessment findings.
• Communicate assessment findings to inform the member's PCP of potential gaps in care.
• Identify needs for clinical referrals and urgent situations, intervening appropriately.
• Educate members on disease processes, medications, and compliance.
