About The Position

As the Hourly Supported Community Living (SCL) Employee you will provide direct support to individuals with disabilities in their homes and community settings. This role focuses on helping individuals live as independently as possible while promoting choice, dignity, safety, and community inclusion. Services are provided according to the individual’s specific service plan and in compliance with Iowa Medicaid and agency guidelines.

Requirements

• To qualify you must be at least 18 years of age and possess a high school diploma or GED.
• You will also need to have reliable transportation and a valid driver’s license in good standing.
• Effective communication skills

• Effective communication and interpersonal skills
• Ability to work independently and make sound decisions
• Dependable and punctual
• Physically able to assist with daily activities as required

Responsibilities

• Support individuals in accessing and participating in community activities (shopping, appointments, social events, recreation, employment, etc.).
• Promote independence, self-advocacy, and skill development based on individual goals.
• Provide supervision to ensure health, safety, and well-being of the individual.
• Assist with medication reminders and/or administer.
• Follow behavior support plans and use positive, respectful support strategies.
• Accurately document services provided, progress toward goals, and any incidents.
• Communicate effectively with supervisors, case managers, families, and team members.
• Maintain confidentiality and comply with HIPAA and Iowa Medicaid requirements.
• Respond appropriately to emergencies and follow agency or individual protocols.
