Responsible for providing prevention and education services in a variety of settings including on the phone. May implement prevention services over a varied group of topics including substance abuse, mental health, bullying, suicide and other areas as necessary. Position Description: Identifies and assesses needs of callers/clients Provide crisis intervention, referrals and linkage to community resources Utilizes supportive techniques and/or educational methods that are appropriate to caller and topic Completes service documentation accurately and timely in electronic health record Provides both prevention and education services Facilitates referral to and/or provides appropriate level of care Develop, define and implement realistic and effective goals and plans for interventions Collect, track and evaluate program data for fidelity and service quality Adhere to professional standards as outlined by protocols, rules and regulations Other duties as assigned This is not a remote position
