About The Position

Mount Sinai Brooklyn seeks a Hospitalist/Palliative Care Physician to care for patients admitted to the Medical Service. Mount Sinai Brooklyn, a 212-bed acute-care community hospital located in Midwood, has gone through numerous renovations and upgrades over the past decade to expand its scope of services. The latest advances include a completely redesigned intensive care unit, a new 128 slice CT scanner, MRI services, new digital mammography equipment, and a new state-of-the-art radiology suite. The hospital’s emergency department also serves as a major hub of activity and a critical community resource. The hospital enjoys an excellent reputation in such specialties as cardiac care, geriatric care, gastroenterology, general surgery, gynecology and orthopedics. Our hospitalists play a central role in enhancing the quality of patient care and medical education. Opportunities are available in quality and patient safety initiatives, house staff and medical student education, and research. Requirements

  • Medical Degree from an Accredited University
  • New York State Medical License
  • Board Eligible or Board Certified in Internal Medicine

Responsibilities

  • Provide comprehensive care to hospitalized patients, overseeing acute medical conditions and developing care plans.
  • Collaborate with interdisciplinary teams comprising nurses, specialists, and social workers to optimize patient outcomes.
  • Conduct daily patient rounds, assess progress, and make informed treatment decisions.
  • Participate in quality improvement initiatives to enhance hospital protocols and elevate patient care standards.
  • Deliver specialized care to patients with serious illnesses, focusing on symptom management, pain relief, and improving quality of life.
  • Conduct thorough assessments to create personalized care plans in collaboration with patients, families, and caregivers.
  • Collaborate with inpatient providers to integrate palliative care into ongoing treatment strategies.
  • Provide support to patients and families navigating complex medical decisions and end-of-life considerations.
  • Lead interdisciplinary meetings to discuss patient care goals and facilitate advance care planning.
  • Ensure continuity of care by managing transitions from hospital to home or hospice care settings.
  • Educate inpatient providers and hospital staff on palliative care principles and best practices.
  • Collaborate with Mount Sinai Health System (MSHS) Palliative Care/Hospice services to introduce innovative programs such as delirium screening and pain management.
  • Deliver two Grand Rounds lectures per year for Mount Sinai Brooklyn.
  • Collaborate with Emergency Department providers to enhance the documentation of goals of care upon admission.
