About The Position

Advanced Practice Provider NP or PA Providing care for all ages Position Summary: Shared role as Scribe to Hospitalist physician as well as APRN in RHC position. Act as a physician's scribe primarily documenting patient encounters, including histories, physical exams, assessments, and plans, into the Electronic Health Record (EHR). Key duties include charting medical information, preparing orders and referrals, and managing administrative tasks, all while strictly adhering to HIPAA and patient rights standards. When acting under the RHC will provide comprehensive health care to patients in collaboration with physician(s) and other medical staff including medical evaluation, counseling and referrals. Assesses the physical & psychosocial status of patients by means of interview, health history, and physical examination and ordering/evaluating diagnostic tests. Interprets the data, develops and implements therapeutic plans, and follows through on a continuum of patient care across the health-illness continuum and life span. Education & Training: Must be a registered nurse, licensed in the State of Illinois, who has successfully completed a formal nurse practitioner program approved by a national accrediting agency and who is then certified by a national certified organization recognized by the Illinois State Board of Nursing. Master’s Degree required. A minimum of 16 hours of Pediatric Continuing Education for hospital-based and primary care Allied Health Care Professionals every two-year period. Experience: Three (3) years of experience as an Advance Nurse Practitioner preferred but not required. An equivalent combination of education and experience from comparable knowledge and abilities may be acquired and will be considered.

Requirements

  • Must be a registered nurse, licensed in the State of Illinois, who has successfully completed a formal nurse practitioner program approved by a national accrediting agency and who is then certified by a national certified organization recognized by the Illinois State Board of Nursing.
  • Master’s Degree required.
  • A minimum of 16 hours of Pediatric Continuing Education for hospital-based and primary care Allied Health Care Professionals every two-year period.
  • Current license to practice professional nursing in the State of Illinois required.
  • Current certification to practice as an Advanced Practice Registered Nurse in the State of Illinois required.
  • Certification as an Advanced Practice Registered Nurse by the American Nurses Association / American Nurses Credentialing Center (ANCC) required.
  • CPR, ACLS, & PALS CERTIFICATION.
  • DEMONSTRATES REQUIRED COMPUTER SKILLS BEFORE THE END OF THE EMPLOYMENT PROBATIONARY PERIOD.
  • WORKING KNOWLEDGE OF CPT-E/M CODING; HCC CODING; HCPC’S; AND ICD-10 CODING PRINCIPLES.
  • Call obligation with a 60-minute response time is required.

Nice To Haves

  • Three (3) years of experience as an Advance Nurse Practitioner preferred but not required.
  • An equivalent combination of education and experience from comparable knowledge and abilities may be acquired and will be considered.

Responsibilities

  • Completes documentation, performing clerical and information technology functions for physician in real-time during patient encounters.
  • Electronic Health Record (EHR) Management: Entering information into the EHR system, including:
  • Patient symptoms and medical history (History of Present Illness – HPI, Review of Systems – ROS, Past Family and Social History - PFSH).
  • Physical exam findings.
  • Provider's assessments, diagnoses, and medical decision-making.
  • Treatment plans and follow-up instructions.
  • Lab results, imaging results, and other test results.
  • Collaborates with physician to establish discharge plan and transitional plan of care as appropriate of each individual patient.
  • In collaboration with physician, evaluates and implements a plan of care addressing patients’ healthcare needs in various settings RHC, LTC, location of patient’s residence). Performs routine exams and obtains patient histories.
  • Manages patient treatment program based on assessment needs.
  • Initiates treatment via prescriptions, diagnostic tests, exams and procedures, as well as coordinates referrals as needed.
  • Review laboratory results and plans actions according to results.
  • Performs routine health maintenance activities.
  • Performs pre-surgical History and Physical exams/Medical Clearance evaluations.
  • Conducts Transitional Care Management (TCM) initial call, as well as TCM follow up visit within 72 hours of discharge.
  • Available for Home Health Telehealth Visits.
  • Performs Cardiac Stress Testing as requested
  • Performs subsequent SWB independently.
  • Performs LTC visits.
  • Teaches wellness habits and educates patients on prevention of potential health problems or methods of dealing with present problems.
  • Provides information and materials relating to patient health care needs.
  • Develops educational programs for to enhance population health upon request.
  • Performs administrative duties as required.
  • Maintains knowledge of current medical practices and skills.
  • Develops, writes, and/or updates Nurse Practitioner practice protocols once a year.
