About The Position
The Hospitalist/Extensivist position at Alignment Health is designed for a licensed Physician who will provide comprehensive medical services to the senior patient population across various settings, primarily focusing on skilled nursing facilities and patients' residences. This role is pivotal in ensuring that high-risk, frail patients receive the necessary care and management they require. The Hospitalist/Extensivist will engage with patients in skilled nursing facilities, ensuring they are seen at least twice a week during their skilled status, which typically lasts about two weeks. Additionally, the physician will be responsible for seeing patients within 72 hours of their admission to the skilled nursing facility, ensuring timely and effective care delivery. In this role, the physician will communicate daily with covering care team members and specialty teams to coordinate care effectively. This includes working closely with community physicians and Alignment care managers to facilitate smooth transitions between different care settings. The Hospitalist/Extensivist will also supervise and mentor Advanced Practice Clinicians, such as Nurse Practitioners and Physician Assistants, as needed, fostering a collaborative environment that enhances patient care. The physician will be expected to create comprehensive and coordinated care plans that incorporate information from medical records, patients, their families, and other healthcare providers. Timely completion of medical records and outpatient visit documentation is essential, as is effective communication with patients and their families regarding care plans and any changes in health conditions. Participation in weekly multidisciplinary meetings for complex case management and being part of the on-call schedule are also key responsibilities of this role. The position requires local travel between care settings and occasional travel to the AHC corporate office, ensuring that the physician remains engaged with the broader organizational goals and care delivery systems.
