About The Position

Provides compassionate and high-quality personal care and prescribed medical treatments to patients at Riverwood Health Care Center, in accordance with the scope of practice for a nursing assistant. Performs all duties under the supervision and delegation of a registered nurse, ensuring adherence to established care plans and facility protocols.

Requirements

  • High School Diploma or GED equivalent preferred.
  • Current and active registration as a Certified Nursing Assistant (CNA) is highly preferred but not required for candidates who have already completed high school or obtained a GED.
  • If currently enrolled in high school, the individual must have an active CNA registration or obtain CNA certification within six (6) months of hire.
  • Basic Life Support (BLS) certification preferred; may be required upon hire based on departmental needs.
  • Current Nursing Assistant (NA) registration with the Minnesota State Board of Nursing highly preferred.
  • Current certification in Basic Life Support (BLS) or ability to obtain during orientation

Nice To Haves

  • Emergency Medical Technician (EMT) certification preferred, but not required.

Responsibilities

  • Delivers safe and compassionate patient care by maintaining a clean, hazard-free environment.
  • Reports near-miss events and actual incidents promptly to appropriate personnel.
  • Provides continuous 1:1 patient observation as assigned to ensure safety and comfort.
  • Manages the unit desk area by answering phones, transferring calls, and responding to call lights.
  • Completes paging requests and communicates messages as needed.
  • Documents patient information accurately in medical records, graphic charts, and flow sheets per policy.
  • Reports significant changes in patient condition promptly to the RN.
  • Shares relevant updates with team members and the charge nurse for coordinated care.
  • Assists with activities of daily living (ADLs) such as bathing, grooming, and toileting.
  • Measures and records vital signs and blood glucose levels as directed.
  • Applies oxygen therapy as instructed by the RN.
  • Supports patient nutrition by delivering trays, setting up meals, and feeding when needed.
  • Monitors and records accurate intake and output.
  • Aids in ambulation and safely transport patients to other departments.
  • Provides non-invasive treatments including applying ice packs and warm compresses.
  • Collects, labels, and transports lab specimens following standard procedures.
  • Restocks medical supplies and maintains the cleanliness of utility rooms.
  • Safeguards patient information and maintains confidentiality in compliance with HIPAA regulations.
  • Assists with patient discharge by gathering belongings and helping the patient to their vehicle.
  • Takes responsibility for professional development by completing required education and training.
  • Demonstrate respectful and courteous behavior with patients, families, and colleagues.
  • Follow all safety protocols and use appropriate safety equipment.
  • Reports any safety hazards to maintain a secure work environment.
