About The Position

Join Our Team as a Hospital Nurse Liaison Hospice of the Piedmont is seeking a compassionate and skilled Hospital Nurse Liaison to serve as a vital connection between hospital teams and hospice care. This full-time position covers the University of Virginia Medical Center and Sentara Martha Jefferson Hospital, ensuring patients and families receive the support and education they need during critical transitions. As the face of Hospice of the Piedmont within these hospitals, you’ll coordinate care for referred individuals, advocate for eligible patients, and provide education to patients, families, and healthcare professionals. This role is ideal for a nurse who thrives on collaboration, patient advocacy, and building strong relationships within the hospital setting.

Requirements

  • Licensed as a Registered Nurse in the Commonwealth of Virginia.
  • Two or more years of relevant nursing experience in terminal care, health team service coordination, documentation and/or case management.
  • Ability to work four 10-hour shifts
  • Ability to manage all aspects of terminal care, health team service coordination, and documentation.
  • Emotionally mature and caring individual willing to give support and assistance to the terminally ill and their families.
  • Intermediate computer skills.
  • A valid driver’s license and current automobile liability insurance.
  • Clinical Knowledge – Hospice care principles, pain management, and regulations.
  • Communication Skills – Clear, empathetic conversations with patients, families, and staff.
  • Collaboration – Work effectively with hospital teams and hospice staff.
  • Patient Advocacy – Support eligible patients and ensure smooth transitions.
  • Organizational Skills – Manage multiple priorities and maintain accurate documentation.
  • Relationship Building – Promote hospice services and maintain strong hospital presence.

Nice To Haves

  • Previous experience in hospice, home health, health care sales or marketing.
  • Strong knowledge of hospice regulations and reimbursement issues surrounding hospice care.

Responsibilities

  • Establish a hospice presence in hospitals by attending rounds, care conferences, in-service meetings, committee meetings, and new staff orientations.
  • Coordinate discharge plans with hospital case managers, patients, families, and hospice staff for newly referred patients, providing consultation and follow-up.
  • Inform hospital patients and their families about Hospice of the Piedmont services.
  • Educate and consult hospital medical professionals on Hospice of the Piedmont services, pain management, hospice care, and reimbursements, advising the Hospital Relations Manager on community needs.
  • Contribute to marketing and business development plans.
  • Ensure census growth.
  • Demonstrate familiarity with agency policies, state, and federal hospice regulations.
  • Transfer patient information from hospital systems to hospice EMR promptly.
  • Advocate for current hospice patients admitted to the hospital or ER.
  • Participate in research, quality improvement, or other end-of-life care studies.
  • Maintain confidentiality regarding patients, families, staff, and the Agency.
