About The Position

The Hospital Medicine Program Manager at Memorial Hermann Health System is responsible for overseeing the clinical workflow of the program, ensuring high-quality patient care and satisfaction. This role involves collaboration with various healthcare professionals, participation in quality improvement initiatives, and management of case planning and discharge processes. The Program Manager will also lead educational programs and maintain strong relationships with both internal and external stakeholders to enhance patient outcomes and operational efficiency.

Requirements

  • Graduate of an accredited School of Professional Nursing; Bachelor of Nursing required.
  • Certified in Basic Life Support; Current State of Texas license or temporary/compact license to practice professional nursing.
  • Minimum of five (5) years nursing experience in an acute care setting.
  • Two (2) years of previous supervisory experience required.
  • Understanding of quality improvement, performance improvement, change management, and patient safety knowledge preferred.
  • Working knowledge of Cerner Electronic Health Record preferred.
  • Proficient in Microsoft Office, including Excel, Word, PowerPoint, and Visio.
  • Strong communication skills, both oral and written, with the ability to convey complex ideas effectively.
  • Ability to work independently and demonstrate excellent problem-solving and critical thinking skills.

Responsibilities

  • Participate in multi-disciplinary rounds and facilitate patient satisfaction escalations.
  • Review quality metrics and conduct quality audits.
  • Monitor dashboards and play an active role in case management discharge planning.
  • Participate in continuous quality improvement with external referral sources and healthcare professionals.
  • Act as the on-site Program Manager, providing guidance and direction to the care team.
  • Collaborate with the Medical Director to oversee clinical workflow and ensure optimal quality of care and patient satisfaction.
  • Serve as a super user for the Electronic Medical Record (EMR) system.
  • Lead in-services, educational programs, and department meetings to keep the team updated on new developments.
  • Develop strong working relationships with external referral sources and internal staff.
  • Implement and evaluate changes in clinical and operational processes.
  • Ensure adherence to Memorial Hermann policies, procedures, and standards within budgetary specifications.
  • Promote individual professional growth and development among staff.
