About The Position

Hospital Liaison – Skilled Nursing Facility Pawtucket Falls Healthcare Center is looking for a Hospital Liaison who serves as the primary point of contact between hospitals and the skilled nursing facility, facilitating patient referrals, strengthening relationships with hospital staff, and supporting the admissions process. This role combines clinical insight, marketing, and coordination to ensure smooth transitions of care.

Requirements

• Experience in healthcare marketing, admissions, or case management (preferred)
• Strong interpersonal and communication skills
• Ability to work independently and manage a flexible schedule, including weekends
• Knowledge of post-acute care services and insurance processes is a plus

Responsibilities

• Conduct on-site hospital visits to evaluate and engage prospective patients for admission to the facility
• Build and maintain strong relationships with hospital case managers, discharge planners, and clinical staff
• Promote the facility’s services and capabilities to increase referral volume
• Coordinate and assist with admissions, including weekend coverage
• Oversee and support patient sign-ins, ensuring accurate documentation and a seamless transition process
• Collaborate with internal teams to confirm appropriate placement and continuity of care
• Track referral activity and provide regular updates to leadership
