  • Bachelor's degree preferred.
  • Experience working with individuals with intellectual and developmental disabilities preferred.
  • CPR Certification.

  • Two years of experience in an acute care hospital setting.
  • Experience in case management or utilization review.

Responsibilities

  • Coordinate and organize nursing services for individuals receiving care in hospitals.
  • Conduct outreach and liaison activities at hospitals and local healthcare facilities.
  • Observe hospitalized individuals and gather pertinent medical information.
  • Provide consultation and training to Direct Support Professionals (DSP) and other staff.
  • Facilitate discharge planning and ensure optimal flow of information.
  • Attend meetings related to the status and care of hospitalized individuals.
  • Review medical records and collaborate to reconcile discrepancies.
  • Complete required reports and documentation regarding hospitalizations.
  • Develop and revise policies and procedures related to hospitalizations.
  • Provide direct care nursing services as needed.
