Works within the Charlotte County hospital system in the effort to assist individuals identified as having substance use or mental health concerns engage in services following medical treatment. Provides practical advice, suggestions, and strategies of recovery to patients as an empowering helper with the perspective of being a recipient of services in the past. Provide one-on-one support. . Work with individuals and families to prevent crisis and promote treatment engagement, while assessing risk ongoing. May help individuals deal with addictions and substance use; family, parenting, and marital problems; suicide; stress management, financial issues; and issues associated with aging and mental and emotional health. Must be very familiar with and actively refer to various social services resources in Charlotte and surrounding counties Provide hope, support, education, and assist patients in setting recovery goals. Provide input, as part of the treatment team, on the patient’s care along with hospital staff Employee will work closely with law enforcement, Department of Children and Families, hospital staff and other local, state and federal service providers to coordinate services for individuals determined to be in need. Employee will make appropriate internal and external referrals, monitor the implementation of services, and provide support and advocacy for the individual. Document in patient record.
