About The Position

The Hospital-Based Violence Intervention Program (HVIP) Social Worker plays a crucial role in supporting patients recovering from gunshot wounds and their families. This position involves providing comprehensive case management, psychosocial support, and advocacy to help patients navigate the complex challenges of recovery. The Social Worker will collaborate with community partners to connect patients with essential resources and ensure effective program delivery.

Requirements

  • Master's degree in Social Work from an accredited institution.
  • Minimum of two years of experience working with at-risk youth or individuals recovering from trauma in a hospital or community setting.
  • LMSW or LCSW required.
  • Experience in providing individual, family, or group therapy.
  • Knowledge of community resources and ability to navigate referral processes.

Nice To Haves

  • Trauma-informed care experience preferred.
  • Bilingual (English/Spanish) a plus.

Responsibilities

  • Conducting comprehensive psychosocial assessments of patients and families to identify their needs, strengths, and resources.
  • Collaboratively developing case plans with patients and families that outline specific interventions and support services.
  • Providing individual and family counseling to address emotional and social challenges related to recovery from gun violence.
  • Connecting patients and families with essential community resources (financial assistance, housing support, job training) through referrals and collaboration with community partners.
  • Advocating for patients and families to ensure they receive the services and support they need.
  • Monitoring patient progress towards case plan goals and documenting outcomes.
  • Identifying and addressing any barriers to successful case management.
  • Participating in team meetings and case staffing discussions to ensure coordinated care and service delivery.
  • Maintaining accurate and detailed electronic medical records (EMR) documentation.
