Position Overview Reporting to the Hospice Director, the Hospital-Based Hospice RN serves as an embedded hospice specialist within the hospital setting. You will provide direct General Inpatient (GIP) hospice care, complete hospice admissions, and act as the primary clinical liaison between hospital teams and Granite VNA. In addition to hospital-based care, this role supports: Same-day hospice admissions in facilities or the community, as needed Backup clinical support to the Hospice House, when required This is a dynamic role for a nurse who enjoys both hands-on clinical care and coordination across care settings. What You’ll Do Complete hospice and GIP admissions, including comprehensive assessments, care planning, and documentation Provide direct nursing care to patients receiving GIP-level hospice services in the hospital Collaborate closely with hospitalists, palliative care teams, nursing staff, case managers, and hospice physicians Educate hospital staff on hospice eligibility, philosophy of care, and symptom management best practices Participate in interdisciplinary team (IDT) meetings to ensure coordinated, holistic care Manage complex symptoms in alignment with hospice protocols and CMS GIP regulations Support transitions of care, including discharge planning and family communication Provide same-day routine hospice admissions in the community or facilities as needed Partner with Hospice House and community teams to ensure continuity of care Serve as a clinical ambassador for Granite VNA, building strong relationships with hospital partners Safely travel throughout the service area as needed using a personal vehicle Who Thrives in This RoleThis role is a great fit for nurses with experience in: Hospice or palliative care ICU, oncology, med-surg, or acute care Admissions, care coordination, or hospital-based nursing If you’re seeking meaningful work, strong team support, and the opportunity to practice nursing at the top of your license—this role offers all of that and more.
