About The Position

Hospital At Home RN SUMMARY (Basic Purpose of the Job) Delivers quality care to patients through the nursing process of assessment, diagnosis, planning, implementation and evaluation within the home setting. Assures patient safety by executing appropriate policies. Serves as an advocate by providing emotional and informational support to patients and families and giving them an opportunity to participate in their plan of care and goal setting. Plans for providing traditional and individualized comfort measures. Works with other healthcare professionals to optimize patient outcomes and customer satisfaction. Educates patients about various medical conditions and provides advice and emotional support to patients' families. Contributes and supports the organization's readiness for various regulatory agencies, Environment of Care, Magnet and various certifications. Hospital At Home Command Center RN SUMMARY (Basic Purpose of the Job) Delivers high-quality acute care to patients receiving hospital-level services in their homes by remotely monitoring clinical status from Capital Health's centralized Command Center. Provides continuous virtual surveillance, clinical assessment, care coordination, education and timely intervention using advanced monitoring technologies and evidence-based nursing practices. Collaborates with clinicians, physicians, and interdisciplinary team members to ensure safe, seamless, and patient-centered care. Serves as an advocate by providing emotional and informational support to patients and families and giving them an opportunity to participate in their plan of care and goal setting. Contributes and supports the organization's readiness for various regulatory agencies, Environment of Care, Magnet and various certifications. Hospital At Home Equipment Tech SUMMARY (Basic Purpose of the Job) Provides support to department with routine, urgent, and complex needs to ensure quality of patient care. Oversees stock, preparation, setup, and maintenance of equipment and supplies in various locations where procedures are performed to ensure quality of patient care.

Requirements

  • Associate's degree or graduate from an accredited school of nursing.
  • Three years experience in an acute care setting.
  • Previous telemetry experience required.
  • AHA ACLS or must obtain within 6 months of hire date
  • AHA BLS - Healthcare Provider
  • Drivers License
  • Registered Nurse – NJ or Registered Nurse – NLC multi-state
  • Requires NIH Stroke Scale Certification (or must obtain by end of orientation) AND 8 hours of Stroke related Continuing Nursing Education (CNE) contact hours annually if assigned to: Critical Care, Intermediate Care Unit, Emergency Department, Neuro Units, Cardiology Inpatient at Hopewell, Peds ED, PACU, Interventional Radiology, CNI, Observation, Hospital at Home

  • Bachelor’s degree in Nursing (BSN) required.
  • Minimum of 3 years clinical RN experience. Preferably acute care, ED, or telehealth.
  • Proficiency in telehealth platforms and electronic health records (e.g., Epic, Cerner).
  • High school diploma or GED.
  • AHA BLS. Non-Licensed must obtain before end of orientation period
  • Knowledge of basic medical terminology.
  • Proficient in English. Verbal and written communication skills.
  • CPR.
  • Ability to communicate with hospital staff.
  • Uses discretion and courtesy when working with staff and visitors.

  • Bachelor's of Science degree in nursing (BSN) preferred.
  • Previous work experience in a medical or healthcare setting preferred.

Responsibilities

  • Delivers quality care to patients through a team effort in working with others through the nursing process of assessment, diagnosis, planning, implementation and evaluation.
  • Assures patient safety by executing appropriate policies & procedures.
  • Operates and troubleshoots remote monitoring devices (telehealth platforms, vitals tracking).
  • Conducts home safety assessments
  • Provides appropriate support for pain management, pharmacological and non-pharmacological measures.
  • Assist patient/family in identifying individualized comfort measures.
  • Plans and provides unique and individualized comfort measures while utilizing intuitive and innovative approaches which are scientifically sound and are a result of evidenced based practice.
  • Mobilizes resources in complex cases to maximize patients control and participation over his/her own recovery.
  • Provides information and interpretation of the patient's condition and offers coping mechanisms. Provides these to both patient and family.
  • Formulates and documents a discharge plan that maximizes the patient's ability to continue with meaningful life activities.
  • Integrates assessment and diagnostic information with intuition to foresee potential age-specific healthcare needs.
  • Anticipates patients needs.
  • Ensures handoff communication and includes a report in terms of the situations most likely to develop and the problems awaiting patient.
  • Identifies proactively issues to be resolved related to patient education for medication management.
  • Provides discharge planning which includes instructions on discharge medications.
  • Uses discretionary judgment to appropriately modify patient care regimens.
  • Facilitates appropriate response from other health care team members to provide quality and safe care.
  • Plans and provides unique and individualized comfort measures while utilizing intuitive and innovative approaches which are scientifically sound and are a result of evidenced based practice.
  • Ensures proper handling and disposal of medical supplies
  • Recognize early signs of clinical deterioration and escalate care promptly (including arranging transfer to brick and mortar hospital if needed).
  • Attends educational sessions when offered in areas that will improve one's ability to assess the needs on one's respective department.
  • Participates in unit based and/or hospital based committees.
  • Participates in performance improvement activities at the unit and/or hospital wide level.
  • Participates actively in the preparation of various regulatory agency readiness.
  • Participates in unit level effort to achieve successful Patient Satisfaction scores and has an awareness of the HCAAPS measurements.
  • Performs any other related duties as required or assigned.
  • Within the on-site command center, conducts virtual nursing assessments via video conferencing technology for patients enrolled in the Hospital at Home program on-site in the command center.
  • Monitors patient status remotely using connected devices and electronic health record system (EHR) data.
  • Provides patient and caregiver education on medications, treatments, and care plans.
  • Collaborates with in-home RNs, providers, and care coordinators to ensure real-time updates and care adjustments.
  • Documents all virtual interactions in the electronic medical record in accordance with NJ Board of Nursing and CMS standards.
  • Supports hospital discharge coordination and virtual onboarding of new patients.
  • Escalates clinical concerns to providers and initiate urgent interventions when necessary.
  • Triages Patient care needs and escalate appropriately.
  • Performs other duties as assigned.
  • Locates, cleans, decontaminates, sets up, and prepares instruments and equipment for the department.
  • Assembles on all necessary supplies.
  • Assists in keeping department clean and orderly as well as keeping the department and department locations stocked with supplies.
  • Demonstrates aseptic technique in all aspects of job duties.
  • Assists with the preparation and maintenance of patient equipment and prevention of waste.
  • Contacts other departments, such as Clinical Equipment, Central Supply, and Maintenance for assistance as appropriate.
  • Demonstrates excellent customer service skills in dealing with patients, visitors, and health care team members.
  • Demonstrates effective interpersonal working relationships and works cooperatively with healthcare team members.
  • Provides indirect patient care and support to the department for routine, urgent, and complex needs to ensure quality of patient care.
  • Checks supplies for par levels, expiration dates, rotation of supplies, and maintenance of supply area for cleanliness.
  • Compiles list of supplies needed and submits to supervisor and manager for ordering.
  • Identifies malfunctioning equipment and notifies appropriate personnel.
  • Performs other duties and projects as assigned.
