About The Position
The Hospital Access Liaison (HAL) assists individuals who have recently been discharged from an Emergency Department (ED) or inpatient unit that need immediate assistance with linkage to services and resources in the community. The focus will be on those individuals who are at high risk of hospitalization/re-hospitalization and/or risk of suicide and need increased services. This is a full-time, benefit eligible position working Monday-Friday, 8:00 am - 5:00 pm. This is a Hybrid position. This position is 4 days co-located in hospital setting and 1 day at home.
