About The Position

Hospice of Marion County a Part of Empath Health is seeking a Hospital - Access Navigator to join our Admissions team. This full-time, leadership role offers an opportunity to make a meaningful difference in patient access and care coordination across our hospital partners. What You'll Do Provide daily leadership, oversight, and support to the Access Team staff within the Admission-Care Navigation Department. Serve as the primary liaison to hospitals, communicating Empath Health's Mission, Vision, and values to partners and stakeholders. Ensure compliance with all state and federal regulations, as well as standards of care established by accrediting agencies. Coordinate and evaluate patient access and care navigation processes to promote quality, efficiency, and continuity of care. What You'll Need Education and/or Experience: A.S. degree required - Bachelor's degree (B.A.) from four-year college or university and Master's degree preferred. Two years Hospice experience with at least two years of management experience preferred. At least five years total years experience as an RN preferred. Licensure: RN Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file. This position requires travel between multiple locations. Requirements

  • A.S. degree required - Bachelor's degree (B.A.) from four-year college or university and Master's degree preferred.
  • Two years Hospice experience with at least two years of management experience preferred.
