The Weekend Hospice Nurse delivers skilled, compassionate patient care during weekends and holidays. As a Hospice Nurse, this role manages clinical needs, coordinates interdisciplinary support, and responds promptly to urgent situations. The Hospice Nurse ensures that patients and families receive consistent, high-quality care in alignment with the hospice plan. Serving as the primary clinical contact during off-hours, the Hospice Nurse is vital in maintaining continuity, comfort, and trust at a critical time in the care journey, fostering emotional resilience and a deep understanding of hospice philosophy and practice. This is a part-time/prn position, and regularly scheduled weekends may not always be available.

  • Serve as the case manager for patients during the weekend coverage period.
  • Conduct skilled nursing visits in alignment with the IDG (Interdisciplinary Group) plan of care.
  • Coordinate client care with social workers, chaplains, in-home aides, physicians, and volunteers.
  • Assess patients and families promptly for changes in condition or urgent needs.
  • Provide client and family education in end-of-life care, medications, and care techniques.
  • Communicate effectively with physicians and the Hospice Administrator regarding patient status.
  • Document accurately and in a timely manner, including medication use, DME, supplies, and care interventions.
  • Participate in scheduled weekend calls, visits, and evaluation visits as needed.
  • Respond to urgent needs, including deaths, symptom crises, or equipment issues.
  • Leave clear documentation and care plans in the home for continuity by weekday staff.
  • Assist in the recertification and revocation processes as needed.
  • Ensure personal and patient safety by adhering to infection control and safety protocols.
  • May participate in orientation or act as a preceptor as assigned.
  • Promote and represent the nursing profession and Healthkeeperz’s mission in a positive light to the community.
  • Active North Carolina RN license
  • ADN required; BSN preferred
  • Minimum one year of hospice or home health experience preferred
  • Excellent assessment, triage, and communication skills
  • Strong organizational skills with ability to work independently
  • Comfortable using mobile devices and electronic documentation platforms
  • Reliable transportation and willingness to travel to patient homes
