About The Position

The Hospice Weekend and After Hours RN position at Goodwin Living is primarily responsible for providing end-of-life care and hospice services to patients in their home or community setting during weekends, after regular business hours, and on holidays. This role involves assessing patient needs, implementing care plans, and ensuring effective communication with patients, families, and the hospice team to facilitate a comfortable and dignified dying process.

Requirements

  • Registered nurse currently licensed to work in Virginia.
  • Minimum 5 years of experience as a professional nurse, preferably in hospice, acute care, or long-term care settings.
  • Certified in Hospice and Palliative Care preferred.
  • Demonstrated ability to assess and respond to the needs of terminally ill residents and families.
  • Positive and professional image and attitude in relationships with patients, families, and peers.
  • Demonstrated skill in nursing practice and coping with family emotional stress.
  • Proficient in Microsoft Word and documentation software.
  • Ability to work independently and demonstrate patient care planning and teaching skills.
  • Strong written and verbal communication skills.
  • Current CPR certification.
  • Valid driver's license and access to a reliable and insured vehicle.
  • Belief in the hospice concept.

Nice To Haves

  • Certified Hospice and Palliative Care certification.

Responsibilities

  • Assess needs and provide support by phone or visit based on patient acuity.
  • Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure, and effective grieving for patients and families.
  • Obtain data on physical, psychological, social, and spiritual factors influencing patient health and incorporate it into care plans.
  • Assist in determining the need to change the level of care for hospice patients in collaboration with the attending physician and hospice medical director.
  • Provide or arrange for delivery of medications and supplies needed after business hours, weekends, or holidays.
  • Provide appropriate support at the time of death, respecting the patient's and family's spiritual and cultural beliefs.
  • Report on-call activities to the hospice team via email at the end of the shift, including communication and changes in condition.
  • Document all clinical nursing visits, medication changes, and care plans in the electronic record within 24 hours of service.
  • Teach or supervise the teaching of household members and facility staff responsible for assisting with nursing or personal care needs.
  • Triage referral calls and assess immediate admission needs for patients.
  • Complete all parts of the admission process, including receiving physician certification if needed.
  • Inform the Director of Clinical Services or Administrator of unusual or potentially problematic resident/family issues per shift.
  • Respond calmly and tactfully in stressful and emergency medical and psychosocial situations.
