About The Position

The registered nurse assesses, plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Requirements

Graduate of National League for Nursing accredited school of nursing.
Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred.
Registered nurse with current licensure to practice professional nursing in the state.
Maintains a current CPR certification.
Self-directed and able to work with minimal supervision.
Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.
Shows ability to organize and prioritize workload independently.

Responsibilities

Participates in on-call duties as defined by the on-call policy, primarily 4:30 pm-8:00 am weekdays and 8:00 am to 8:00 am on weekends & holidays.
Assesses, evaluates patient symptoms or concerns and instructs in interventions as appropriate.
Evaluates need for follow up visit based on patient status, reports of symptoms.
Communicates need for follow up home visits to the field nurse (LPN/RN) on call.
Documents triage assessment and interventions, follow up in the patient medical record.
Communicates with patient team members to notify them of the after-hours patient call, and results of call.
Enters appropriate order for scheduling visits as appropriate.
Process order via workflow.
Assigns PRN visit to on-call field clinician to perform.
Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
Communicates with community health related persons to coordinate the care plan.
