Requirements

  • NP / MSN (or advanced practitioner degree) or equivalent PA degree.
  • Current NP CA License.
  • Board Certification or eligibility in internal medicine, pediatrics, or family medicine.
  • Exceptional communication skills for impactful patient and team interactions.

Responsibilities

  • Serve as the physician designee when the Medical Director is not available pursuant to a mutually agreed upon schedule.
  • Be available to Hospice staff for clinical guidance and patient care needs.
  • Review and document the clinical information for each Hospice patient and provide written certification of life expectancy.
  • Identify primary terminal condition and all related and unrelated diagnoses.
  • Review current subjective and objective medical findings.
  • Identify medication regimen and determine which medications are hospice related/covered.
  • Review recommended treatment interventions per attending MD and provide input into appropriateness related to terminal illness and Hospice plan of care.
  • Work closely with Utilization Review Department to identify appropriate ICD 10 codes.
  • Review patient's clinical information before recertification and determine continued eligibility in collaboration with attending MD.
  • Establish the plan of care in conjunction with the attending physician and interdisciplinary team.
  • Review and sign C2 (Controlled Drugs) prescriptions as required by law.
  • Review and sign Death Certificates ensuring accuracy of information.
  • Facilitate communication between medical staff and Hospice staff as needed.
  • Make home visits with Hospice staff for clinical guidance and changes in the Hospice Plan of Care.
  • Assume attending physician role for patients on General In-Patient level of Care in the Hospice homes and Sharp Hospitals.
  • Document in the Electronic Health Record per agency protocol.
  • Perform Face-to-face encounter visits in accordance with Medicare requirements and Hospice policy.
  • Participate in educating Hospice staff through formal and informal in-services.
  • Participate in Case Conference meetings to review active cases as requested.
