Team Coordinators are central to operations, ensuring smooth care delivery for patients and families. This role involves organizing, accurately managing, and timely processing the flow of care, including scheduling, communication, document management, and supply coordination. The Team Coordinator's attention to detail and commitment to excellence enables nurses, social workers, chaplains, and providers to focus on patient support. As the administrative backbone of the interdisciplinary team, the Team Coordinator manages schedules, organizes communication, and ensures clinical documentation is accurate, timely, and accessible. Responsibilities include preparing for meetings, maintaining patient records, coordinating supplies and equipment, and responding quickly to needs. This role provides structure, keeping the team aligned and empowered to serve patients with excellence, thereby protecting the continuity of care and supporting the mission of helping people live life to the fullest.

Requirements

  • High school diploma or equivalent required
  • Excellent interpersonal and organizational skills with strong verbal and written communication.
  • Proven ability to prioritize tasks, make decisions, and solve problems in a fast-paced environment.
  • Comfortable working both independently and as part of a team.
  • Basic proficiency in computer systems
  • Valid driver’s license, reliable transportation, and auto liability insurance.
  • Must be able to read, write, and communicate clearly in English.

Responsibilities

  • Support communication flow – Answer phones, triage messages, and ensure timely follow-up by routing communication to the appropriate team members.
  • Manage clinical documentation – Upload and distribute physician orders, history and physicals (H&Ps), and other clinical records in the EMR system to support seamless care.
  • Coordinate team meetings – Prepare agendas, distribute notes, and support facilitation of Interdisciplinary Team (IDT) meetings.
  • Maintain patient records – Assemble and update patient binders and ensure necessary materials are available for facility visits and team conferences.
  • Coordinate durable medical equipment (DME) – Facilitate ordering, delivery, repair, and pickup of DME to meet patient needs in the home or facility setting.
  • Notify pharmacy of key updates – Alert pharmacy to all patient admissions and deaths to support timely medication management and billing accuracy.
  • Manage supply logistics – Track, order, and distribute medical and office supplies to ensure field staff are equipped for care delivery.
  • Assist with administrative tasks – Support day-to-day operational needs, including file maintenance, tracking, and additional duties as assigned.
  • Adapt and respond – Remain flexible in supporting changing team and patient needs, with a willingness to adjust priorities throughout the day.
