The Hospice Team Coordinator is responsible for maintaining staff schedules, scheduling patients for admission and home health aide visits, and coordinating additional staffing resources when needed. This role requires detailed knowledge of the coverage area for each staffing resource/vendor and any restrictions or limitations for each resource/vendor. The coordinator is also responsible for ensuring timely execution of support functions to meet required regulations, monitoring specified Epic reports, and preparing for weekly IDT meetings. The position primarily involves telework, with occasional on-site presence required for training.
