What you’d do as the Hospice Spiritual Care Coordinator: Arrange for counseling or provide counseling to meet the spiritual needs of patients/families/caregivers in a manner consistent with their beliefs and desires. Provide comprehensive assessment of the patient, family, and caregiver’s spiritual needs. Complete bereavement risk assessments of the patient/family/caregiver and determine the bereavement needs. Identify appropriate spiritual and bereavement interventions and goals for the patients care plan. Participate as a member of the interdisciplinary team (IDT) in the development of patient care plan. Ensure the services that are provided are in accordance with the patient’s acceptance of these services and the patient’s care plan.
