Empyrean Hospice hires Spiritual Care Coordinators for the provision of spiritual care services to patients and families/caregivers of Empyrean Hospice, either directly or through coordination of care with other spiritual counselors. The position requires representing Empyrean Hospice with professionalism and compassion, providing spiritual assessments, facilitating individualized care plans, and serving as a member of the interdisciplinary group. The coordinator will also provide direct spiritual care, maintain records, recruit community chaplains, and plan memorial services as needed. The role emphasizes ethical practice and community engagement.

  • Represent Empyrean Hospice with the utmost professionalism and compassion
  • Provide spiritual assessment of patients and families/caregivers in the hospice program
  • Facilitate the development of the individualized plan of care
  • Serve as a member of the interdisciplinary group and provide consultation, education, and support
  • Provide direct spiritual care to patients and families/caregivers
  • Serve as liaison and support to community chaplains and spiritual counselors
  • Maintain records of spiritual care services utilization and related activities
  • Recruit community chaplains, spiritual counselors, and spiritual care volunteers
  • Provide for funeral or memorial services for patients as requested
  • Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors
  • Assist in supervision of spiritual care volunteers when assigned
  • Ensure standards of ethical business and clinical practice are prioritized
  • Professionalism, compassion, empathy, and a desire to help others
  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree)
  • Experience working with death and dying, individuals/family/caregiver
  • Minimum of 1 CPE required
  • Self-directing with the ability to work with little direct supervision
  • Understand the philosophy of hospice care and of grief response
  • Demonstrate comfort in an interfaith setting
  • Ability to network with community clergy and congregations
  • Possess strong communication skills, verbal and written, and organization skills
  • Must be a licensed driver with an automobile that is insured
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
