Identifies functional speech defects and establishes a plan of care to improve patients function. Administer diagnostic speech and language evaluations and some basic, informal audio logical assessment in accordance with the plan of care Recommends communication devices/aids as indicated. Prepare a report of each patients diagnostic evaluation and make recommendations Advise, consult, and teach appropriate procedures and supportive activities to patient, family members and other agency staff Provide direct speech service to patient under a providers plan of care. Records and reports to the provider of the patients response to therapy Participate in the development of the plan of care Assist provider in evaluating the level of function and developing a plan of care As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care Identifies patient and family/caregiver needs for other services and refers as appropriate. Serve on the clinical record review committee Maintains accurate clinical records, keeping track of goals and progress of patient Abides by all regulations, policies, procedures and standards Teaches clients how to properly use therapy techniques

Requirements

  • Possess certification of clinical competence in speech from the American Speech Language Hearing Association
  • Licensed as a speech therapist in the state in which practicing
  • At least one (1) year acute care setting or equivalent experience
  • Possess knowledge of principles and practices of speech therapy
  • Complies and maintains current CPR certification.
  • Ability to make home visits and provide nursing care in the home environment
  • Ability to complete necessary documentation appropriately, legibly and timely
  • Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
