About The Position

The Hospice Social Worker at Athena Home Health & Hospice plays a crucial role in providing emotional and social support to patients and their families during terminal illness. This position involves assessing psychosocial needs, developing care plans, and collaborating with an interdisciplinary team to ensure comprehensive care. The role emphasizes the importance of understanding the unique cultural and emotional needs of patients, while also providing crisis intervention and education on advanced directives.

Requirements

  • Master's in social work required.
  • Experience in health care and hospice preferred.
  • Must be a licensed driver with an insured automobile.
  • Ability to read, write, speak, and understand the English language.
  • Ability to make independent decisions when circumstances warrant such action.
  • Ability to deal tactfully with personnel, residents, family members, and the general public.
  • Possess patience, tact, cheerful disposition, and enthusiasm.

  • Understanding of hospice philosophy and issues of death/dying preferred.

Responsibilities

  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.
  • Carry out social evaluations, including family dynamics and caregiver abilities, and plan interventions based on findings.
  • Counsel patients and families/caregivers regarding stress and coping difficulties, providing crisis intervention when necessary.
  • Educate the interdisciplinary group on any special needs related to the culture of the patient and family.
  • Maintain clinical records on all patients referred to social work.
  • Educate patients and families on advanced directives and assist in their preparation.
  • Provide information and referral services for practical and environmental needs of patients and families/caregivers.
  • Serve as a liaison between patients, families/caregivers, and community agencies.
  • Maintain collaborative relationships with organization personnel to support patient care.
  • Participate in the development of individualized plans of care and attend interdisciplinary group meetings.
  • Assist team members in understanding significant social and emotional factors related to health problems and death/dying issues.
  • Participate in quality assessment performance improvement teams and activities.
  • Assist families and patients in planning for funeral arrangements and decision responsibilities.
