This role involves providing medical social services to patients and their families according to the Plan of Care. Key duties include conducting a psychosocial assessment within 5 days of admission to the hospice program, covering emotional, social, financial, and environmental resources, and identifying appropriate psychosocial problems, interventions, and goals. The social worker will participate in IDG meetings and the development of patient care plans, observe, record, and report changes in the patient's emotional and social factors that are being affected by the patient's illness and his/her need for care and response to treatment. The social worker is also responsible for maintaining and submitting written clinical records as deemed by Care 360 Hospice, including the initial evaluation, the care plan, and daily notes. Weekly scheduling of visits with the patient's case managers is required to facilitate coordination of other staff visits. The role also involves assessing the patient's and family member's ability to cope with the patient's passing and acting as a consultant to the members of the health team, assisting them in understanding the social, emotional, and environmental factors related to the patient's health problems. All other duties as assigned.
