Requirements

Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education
Current Social Work licensed in the state(s) of practice (Certified Social Worker (CSW), Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), Licensed Clinical Social Worker (LCSW), Social Work Training Certificate (SWTC))
Ability to pass background, consumer report, and drug test
Must have reliable transportation, valid driver’s license, and proof of auto insurance

Responsibilities

Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families
Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities
Collaborate with the interdisciplinary team
Maintain accurate documentation
Support cultural and end of life needs
Liaise with community agencies
Ensure HIPAA compliance
