Summary/objective: Social Worker shall help patients and their families to enhance or restore their capacity for optimal psychological, emotional, spiritual, social, and physical health. They identify coping skills and strengths utilized by the patient and family. Social Workers function as an integral part of the interdisciplinary team by addressing the psychosocial stressors and collaborating with the team to enhance the quality of end-of-life care for the patient and their family. Essential Functions: • Complete an initial psychosocial evaluation within the time limits set by the Conditions of Participation and/or state regulations. • Demonstrate a working knowledge of the Conditions of Participation and state regulations governing hospice care. • As allowed by the patient and family, assist with advanced care planning to ensure that all treatments meet the wishes of the patient receiving care. Advanced care planning can include advanced directives, DNRs, DPOAs, financial planning, and funeral planning. • Assist the patient and family to understand, accept, and follow medical recommendations and provide services planned to assist the patient and family in achieving the optimum social and health adjustment within their capacity. • Utilize available resources to assist the patient and family to live better within the limitations of illness. • Records and reports observed social and emotional changes in a timely manner. • Demonstrates the ability to prioritize and balance daily schedules and complete duties in appropriate time frame. • Complete tasks independently. • Educate, mediate and problem-solve with patients and families regarding ethical dilemmas that may arise. • Helps to foster a collaborative culture by communicating consistently and effectively about identified needs and tasks. • Participates in education program by attending required training. Maintains skills necessary for job responsibilities and seek assistance when appropriate. Supervisory responsibilities: This position does not involve any direct supervisory responsibilities. Budgetary/Fiscal Responsibility: Responsible for using resources wisely to support cost-effective operations. Compliance Responsibility: As an essential function, this position is responsible for complying with the Compliance Program as it applies to the individual job duties, the department, and the company. This position will exercise due diligence to prevent, detect, and report unlawful and/or unethical conduct by fellow co-workers, professional affiliates, and/or agents. Policy and Procedure Interpretation and Development: Responsible for complying with all company and clinical policies and procedures. Participates in the audit process as requested to ensure compliance with policies and procedures. Maintains confidentiality of privileged information and supports the employee’s and patient’s right to privacy. Work Environment/Schedule/Travel: Routine schedule during standard business hours. . Requires frequent travel to and work in patients’ homes within the hospice’s service area, and attendance at meetings in office as needed. Proficiency in using standard Microsoft/office software and applications (e.g., email, word processing, spreadsheets) is essential along with basic data entry skills, as well as the ability to quickly learn and adapt to new software and systems used in our office. • Flexibility to adjust to changing priorities, tasks, and environments.
