Job Description Full-time Hospice Social Worker Position Schedule: Monday thru Friday, 8a-5p Available territory openings: Social Worker assigned to counties surrounding nearest office

Requirements

  • One year Hospice experience is preferred, but not required
  • One year of healthcare experience is required
  • MSW required at a minimum
  • Licensed Social Worker in State of Indiana, (LSW/LCSW)
  • Current driver's license
  • Must have reliable transportation and insurance

Responsibilities

  • Assess the psychosocial/spiritual needs of patient/family
  • Provide social service activities as related to terminally ill patients and their families, including planning, treatment and consultation with other staff
  • Assist with funeral planning
  • Assist in the development and implementation of the patient/family Plan of Care
  • Complete the Initial Psychosocial Assessment within 5 calendar days of patients admission
  • Provide counseling of patients/families and hospice staff as needed
  • Participate in bereavement follow-up services as required
  • Maintain adequate and complete documentation of all activities and services provided to patient/families
  • Assist in the development of support groups and other special programs as necessary
  • Facilitates respite and long term care placement
  • Provides referrals to community services as appropriate
  • Works as a team member
  • Participate in weekend on-call rotation
  • Other duties as assigned
