Providence Hospice is seeking a Hospice Social Worker, PRN in St. John, IN. We are looking for a compassionate Social Worker to join our established, experienced hospice team. Ordering Durable Medical Equipment and planning discharges from hospitals and/or nursing homes. Visiting patients and families on a weekly, bi-monthly, or monthly basis to provide ongoing counseling and support as patients are transitioning and dealing with end of life issues.
Requirements
- Licensed Social Worker in the State of Indiana.
- Minimum of (1) year Home Health and Hospice experience preferred.
- Pass a physical, drug screen, 50-pound lift test, fingerprint for criminal background and complete the orientation program.
