As a Social Worker supporting our Broward County program, you will provide meaningful psychosocial support to patients and families navigating serious illness and end of life. This role blends presence, advocacy, and practical guidance, offering support through complex emotions, changing needs, and important decisions. This position follows a Monday through Friday schedule from 8:00 a.m. to 5:00 p.m. and may include participation in on call support as needed, while working alongside an interdisciplinary team to ensure care is compassionate, inclusive, and centered on what matters most to each individual and family. Overview The Social Worker provides emotional support, counseling, and resource navigation to patients, families, and caregivers as part of the interdisciplinary hospice team. This role collaborates with nursing, medical, and spiritual care staff to develop and implement individualized plans of care aligned with patient goals and values. The Social Worker also supports bereavement coordination and may provide supervision for other social work team members in accordance with licensing requirements.
